Target Case Management

Case Management assists individuals who are eligible for services from the Division of Mental Retardation and Developmental Disabilities in accessing comprehensive medical, social, educational, and other specialized services.  Service Coordinators employed by the Regional Office, by a county SB 40 Board or Affiliated Community Service Provider (ACSP) are professionals who are trained in the field of Mental Health and/or closely related fields.  Service Coordinators provide "case management" and are sometimes also referred to as "Case Managers."  Each individual who is found eligible for services from the Regional Office is assigned a specific Service Coordinator who will become acquainted with the individual, and work with him or her to identify, locate, coordinate and monitor the services that meet the individual's particular needs. 

In September 2007, Ray County Board of Services hired its first Service Coordinator and in less than one year doubled its case load and hired a second service coordinator.  Today, there are 4 total Service Coordinators servicing Ray County.
